I am 5’6 and normally wear a medium. I did order a medium and it’s a loose fit but very comfortable. It’s not really an off the shoulder shirt, yes the neck line is bigger and you can wear it off the shoulder if you pull it down.
The 7/8 is closer to a 4/5 fit

I am 5’3 and 140 pounds with DD bust. I normally wear small or medium and went with medium to give it a looser fit. It was perfect. I love it and can’t wait to order another color!!! The fabric is soft.
